# Risk Log

## What it does
Consolidates the scattered open items (risks, RFIs, issues, client decisions) into a single register, ranks them, and makes the ownership and the deadline explicit on every line.

## When to use
- Monthly, as the risk and issues section.
- Before any client meeting where decisions are needed.

## Instructions
1. Ask for: the current risk register, the RFI log, the issues log, meeting minutes for the period, and the programme.
2. Merge into one register with: reference, description, category, owner (a named person or organisation, never "team"), raised date, required-by date, status.
3. Score each item for likelihood and impact, and give a combined rating. State the scale used.
4. Link every risk with a time impact to the affected programme activity from the programme skill.
5. Sort so that overdue items and items with a required-by date inside the next month come first.
6. Separate items awaiting a client decision into their own list, with the consequence of a late decision stated in one line each.
7. Close out anything resolved this month and say how it was resolved.

## Output format
- Risk register, sorted by urgency then rating
- Overdue items, called out separately
- Awaiting client decision: item, required-by date, consequence if late
- Closed this month, with the resolution
- Movement summary: opened, closed, still open

## The one mistake to avoid
Owners recorded as a company or a team rather than a person. An unowned risk is not managed, and the register becomes a list you copy forward each month without acting on.
